Dear Parent or Guardian,
I would like to take this opportunity at the beginning of the semester to introduce myself and the health education program. I am very excited to be beginning the new school year! I am looking forward to getting to know each child throughout the semester in this class.
For the next eighteen weeks, I plan on focusing on various health topics and how they relate to each child on a personal level. At the same time, I hope to foster self-esteem, cooperation, and responsibility through various class activities and projects.
As a practical living teacher, I try to make all class activities/assignments as "real world" as possible. For example, students will be completing a nutrition plan, learning refusal skills, learning what to do in case of a disaster and emergency, and participating in the "Why kNOw?" which is an abstinence only program.
A new opportunity has arisen in health class this year. All students will be training in CPR and will have the opportunity to become certified by the American Heart Association for CPR. This new opportunity has come with a small cost of $5.00. This class fee will cover the portion of class that can result in the certification of each student in class to become certified in CPR. The cost includes: wipes, AHA CPR booklet, replacement lungs, face inserts, and certification cards if the students are eligible (passes the skills test and the AHA certification test. This fee is due by August 25, 2008.
To be successful, students will need to be prepared EVERY DAY! Successful students bring paper, pencil, a pocket folder or binder, and a positive attitude to class. Homework will be collected at the beginning of class. If a student does not turn in his or her homework on its due date an automatic 20% will be taken off the grade once the homework is completed and turned in. Students will be encouraged to use his or her Tiger Book to write down all assignments.
No students will be permitted to leave health class the first 10 minutes or the last 10 minutes of class (10/10 rule). Students will only receive one hall pass per month in health class. A student MUST complete the WCMS hall pass and have it signed by the teacher along with my initialing of his/her OWN TIGER BOOK. If a student does not have his/her Tiger Book he/ she will NOT be permitted out of class.
If a student is tardy to class, documentation will occur. Upon 3 tardies an office referral is completed.
The consequences for the WCMS Health Class**;
1st-Verbal Warning
2nd-Parent Call
3rd-Office Referral
**Once grade level consequences have been established Mrs. Hall reserved the right to follow the grade level system instead of the consequences listed above.
A student's grade will be the average of the total possible points earned for the class. The following grade scale will be used:
A 92-100
B 83-91
C 74-82
D 65-73
F 64 and below
Flashbacks, bellringers, pop quizzes, in class assignments, projects/activities, and homework are all key components of the overall class grade.
If a student has an excused absence, it is his or her responsibility to make up any missed assignments. Each student will need to ask at the end of class for any make-up work he/she needs for the time missed. Students will have one week to make up all missing work from an excused absence. A table will be located near the classroom door where all assignments for the week for each class will be left for absent students to pick up all handouts and assignments.
There are several opportunities to play a key role in helping me educate the students. I would like to have a great deal of community support and involvement. Please let me know if you have any suggestions. Together we can make this an excited and education semester!

Your child will receive credit for turning in this assignment or will receive a zero for the assignment if he/she does not turn in the signed form.
